Pentosan polysulfate: a simple guide

What it is
Pentosan polysulfate (PPS) is a prescription medicine also sold as Elmiron and under other brand names. It’s a sulfated sugar compound used to treat bladder problems.

What it’s used for
- In humans, PPS is used to relieve bladder pain or discomfort from interstitial cystitis.
- The evidence for benefit is mixed, and doctors often reassess after about six months. If there’s no improvement, stopping the medication is usually recommended.
- It was approved for medical use in the United States in 1996.

How it works
PPS is thought to mimic the bladder’s natural lining, forming a protective coating on the bladder wall. This coating may help reduce irritation and the bladder’s permeability.

What you should know about effectiveness
- Some people experience relief, but results are not consistent across all patients.
- Because responses vary, doctors weigh benefits and risks case by case.

Side effects and safety
- Common issues: gastrointestinal problems (diarrhea, heartburn, stomach pain), hair loss, headaches, rash, and insomnia.
- Because PPS can affect blood clotting, some people bruise more easily. It may need to be paused before major surgery.

Eye health: a potential serious side effect
- PPS can be linked to a condition called PPS maculopathy (PPSM), which affects the retina and vision.
- Risk increases with the total amount of PPS taken; even after stopping the drug, about 1 in 5 people may still see progression.
- Regular eye exams and retinal imaging are important for people who have used PPS.
- Current guidance: use the lowest effective dose and monitor closely for early signs of eye damage.

Other uses and research
- PPS has been studied for osteoarthritis and other conditions, but strong, consistent proof of benefit is lacking.

Veterinary uses
- PPS is also used in animals. In dogs with osteoarthritis, results are mixed.
- In horses, a product called Zycosan was approved by the U.S. FDA in 2022 for osteoarthritis and is given by injection. This use is for horses only and not for people.

Brand names and identifiers
- Besides Elmiron, PPS is known by many synonyms and brand names around the world.

Bottom line
Pentosan polysulfate is a long-used medicine for interstitial cystitis with mixed evidence of benefit. It carries a notable risk of eye damage with long-term use, so dosing and regular monitoring are important. It’s also used in veterinary medicine, particularly in horses.
